OTA Volunteer Forms

Thank you for wanting to become an OTA Volunteer.  We truly appreciate the effort and time that you dedicate to our school.  Please keep in mind that to volunteer, you must complete the following:

1. Student Privacy and Volunteer Confidentiality Form

2. Live Scan Form(appointments need to be made, info is on the form)

3. Registered Volunteer Fingerprint Criminal Background Check Authorization

4. Present a Tuberculosis Clearance Card
